Descendency Forms

You may submit a Descendency Form by following these steps:

Open the Descendency Form.  Save it to your computer desktop.

Type in the required information.

Save the document and/or print a copy for yourself.

Either print another copy and mail it to:

or

Attach the .pdf to an email and sent it to hal@edgefalls.com.
